Participate in decisions about your care and treatment.
Receive respectful and dignified care that respects your culture, language, and beliefs.
Receive services free from discrimination.
Refuse medications, tests, or treatments and be informed of risks and benefits.
Know the names and credentials of professionals providing services.
Receive services that meet your needs.
Be referred if the agency cannot meet your needs.
Receive care in the least restrictive and safe environment.
Be free from abuse, neglect, and improper restraint or seclusion.
Have your records kept confidential according to federal and state law.
Have your substance use disorder records protected under federal law (42 CFR Part 2).
Receive a copy of our Notice of Privacy Practices.
Create medical or psychiatric advance directives.
Request another provider when available.
If you believe your rights have been violated, you may file a complaint with the Colorado Behavioral Health Administration (BHA): https://bha.colorado.gov/help/submit-a-complaint
You may also file a privacy complaint with our Privacy Officer, Jennifer Abbie, at 303-981-7868 or jen@asacredjourney.org, or with the U.S. Department of Health and Human Services, Office for Civil Rights, at https://ocrportal.hhs.gov/ocr/smartscreen/main.jsf. We will not retaliate against you for filing a complaint.
